Please follow these directions closely so that we can ensure your teams are registered!

Company Name (if applicable), email address, physical address, phone number.
How many teams you plan to enter according to your sponsorship level (donation only, 1, or 2)
The names of the 4 players for each team entered
If you paid by credit card using the ParentSquare Pay link attached to this communication (recommended)
If you are choosing to pay by check made out to the MFEE (Please let us know if you need an invoice and/or receipt. We will send it to the address provided.)
